de praktijk zijn aanwezig<\/li>\n                            <li>Reden van verwijzing met vermoedelijke DSM 5 classificatie<\/li>\n                            <li>Datum van verwijzing<\/li>\n                            <li>Handtekening van de verwijzer of praktijk stempel<\/li>\n                            <li>Gegevens van de cli&euml;nt: BSN, verzekeringsnummer en contact informatie<\/li>\n                          <\/ul>\n                        <\/div>\n                      <\/div>\n                      <div data-colibri-id=\"1353-c38\" class=\"h-text h-text-component style-1924
